GC Adolescent Autism Support are specialists in level 2 NDIS plan managed and self managed support coordination and access to the community (support work) that is tailored specifically to meet the needs of neurodiverse adolescents. We are more than support coordinators and support workers, we use professional knowledge, skills and lived experience to offer a strengths based approach. We are here to partner with clients and families to connect to qualified and specialised service providers and support workers who match each client's individual strengths, goals and transition pathways.
By doing this, we can help reduce complexity in the participant's support environment, overcome immediate and significant barriers to plan implementation, move towards independence, connect with the community and ultimately improve the participant's overall engagement and goal achievement.
At Gold Coast Adolescent Autism Support, we are committed to providing the highest quality of support to enable participants to achieve their goals and to maximise the value and impact of the
funding they receive from the NDIS. We think outside the box to provide highly individualised support coordination and will address complex barriers client's experience when planning and accessing supports.
We guide clients to;
· maximise the use of their NDIS plan
· make informed decisions and therefore genuinely exercise their right of choice and control
· build their own capacity to manage their plan
· have the opportunity to explore and connect with their community and other support options
· access better coordination between their NDIS plan goals and service providers
Navigating the transition from adolescence to adulthood can be challenging. GC Adolescent Autism Support provides comprehensive level 2 NDIS support coordination to ensure clients are connected to impactful NDIS support services centered around life skills, education, employment, social emotional wellbeing, connection to the community and the client's identity.
We develop customised support coordination plans designed using a deep understanding of the cognitive development during adolescence and the unique needs and goals of neurodiverse adolescents. We support young people to create a clear path to success and independence.
This approach to support coordination ensures the client receives individualised support that fits and will empower and equip them with the tools to transition to adulthood.
